Insulin pump therapy follow-up

The pump setting and use data is uploaded to cloud and then downloaded onto a computer, and it can be viewed on the computer or printed out.

It is advisable to periodically review the information obtained from the pump and glucose monitoring yourself. The unit responsible for pump therapy typically uses device-specific extraction systems or cloud service. The prevailing practice is that the pump user preloads the data at home to a cloud server.

From the pump data, the following are examined:

  • how often the cannula is changed,

  • the total daily amount of insulin and carbohydrates and their variation,

  • the proportions of basal insulin and mealtime insulin,

  • the need for corrective insulin,

  • the accuracy of the dosing guide's suggestions.

The program calculates the average blood sugar from the blood glucose measurements or glucose sensor monitoring, the time spent within the target blood sugar range, indicators related to the variation of blood sugar, and the estimated A1c value. In glucose sensor monitoring, this is referred to by the abbreviation GMI, or glucose management index.

Particular attention is paid to the following during follow-up:

  • How does blood sugar change overnight, i.e., is the night time basal appropriate?

  • What is the blood sugar usually before different meals?

  • What is the blood sugar usually 2–4 hours after different meals?

  • How large a proportion of glucose values are below 3.9 or below 3.0 mmol/l?

  • In what situations do values below the target level occur temporarily or repeatedly?

  • Have there been serious hypoglycemias requiring the assistance of a bystander or emergency services?

  • How large a proportion of glucose values are above 10 mmol/l or above 14 mmol/l?

  • In what situations do values above the target level occur temporarily or repeatedly?

  • Has there been diabetic ketoacidosis (DKA)?

  • How successful has the insulin dosing been during physical activities?

  • Has there been the courage to try out the pump's various functions in changing situations?

  • How has the pump helped in daily life?
