Pal­lia­tive care and hos­pice care ser­vices in Central Uusi­maa Well­being Ser­vices Coun­ty

The wellbeing services county of Central Uusimaa (Keusote), comprises six municipalities (Hyvinkää, Järvenpää, Mäntsälä, Tuusula, Nurmijärvi, and Pornainen), with a total of about 200,000 inhabitants. The hospital in the area is HUS Hyvinkää Hospital. Palliative care in region is provided in collaboration with the Palliative Care Center, acute wards, home care, assisted living services, and, when necessary, other units.

The Palliative Care Center of Keusote consists of a palliative outpatient clinic, a regional home hospital, and a palliative ward (team). The center has training rights. The center works in a multidisciplinary manner and also provides psychosocial support services for patients and their families. Available services include, among others, a health social worker and a hospital chaplain. The Palliative Care Center is responsible for special-level (B-level) palliative care in the region and supports basic-level and A-level units in delivering palliative care.

Demanding special-level hospice care is provided at Terhokoti in Helsinki.

The palliative outpatient clinic is located on the fourth floor of Hyvinkää H Hospital during weekdays during office hours. Patients also have the opportunity to contact a nurse by phone outside office hours.

Access to the clinic is by physician referral. The unit is staffed by a specialist physician and a nurse with palliative care specialization. Palliative patients from all disease groups are treated at the clinic. The services include, among other things, planning of symptom-relieving medication, procedures, and preparation of care plans.

HUS Hyvinkää Hospital also provides palliative consultation, which assists in the care of palliative and end-of-life patients treated in wards and coordinates the continuation of patient care.

Region has a 24/7 at-home hospital service that brings palliative care directly to the patient’s home and also enables end-of-life care at home. Access to the at-home hospital is by physician referral, and the care provided is at hospital level in the home setting.

The at-home hospital operates regionally from four units (northeast, southeast, southwest, northwest). Each unit has a physician with palliative care qualifications and staff who provide multidisciplinary care for the patient and family. Long-term care and support are provided by home care services. The acute wards in the area serve as support wards for palliative patients of the at-home hospital, and admission is always based on a professional assessment.

The at-home hospital also supports the implementation of palliative and end-of-life care in other local units, such as assisted living facilities and home care. This ensures that the final stage of life can take place in a familiar environment.

Hyvinkää H Hospital houses Keusote’s special level palliative care ward with 8 beds.

The ward provides care for all patients in the area who require special level treatment. The staff is trained in palliative care. Volunteers are also involved in the daily activities of the ward, organizing various programs.

Family members have the opportunity to support the patient on the ward around the clock. The patient’s care and daily routine are arranged as individually as possible according to their needs and wishes.

The acute care ward provides round-the-clock inpatient hospital care. Within the Keusote area, there are four different facilities where hospital care can be delivered. These units are located at H Hospital adjacent to Hyvinkää Hospital, JUST in Järvenpää, Mäntsälä Health Center, and Nurmijärvi Kirkonkylä Health Center.

Palliative care is provided in all acute care wards, where the needs of the patient and family are taken into account as comprehensively as possible. Relatives have the opportunity to participate in the palliative care process.
