Pal­lia­tive care and hos­pice care ser­vices in the Well­being Ser­vices Coun­ty of Pirkanmaa

The Wellbeing Services County of Pirkanmaa has approximately 540,000 inhabitants. Palliative care is provided in collaboration with the Palliative Care Center, hospital-at-home services, local hospital wards, assisted living services, and other units when necessary.

The Pirkanmaa Palliative Care Center provides specialist‑level palliative care and consultations, and coordinates the palliative care service pathways within the Wellbeing Services County of Pirkanmaa and the Inner Finland cooperation area.

Pirkanmaa Palliative Care Center

The Pirkanmaa Palliative Care Center forms a joint area of responsibility together with the Pirkanmaa Hospital-at-Home services. The Palliative Care Center includes: the palliative unit of Tays Central Hospital (outpatient clinic and consultation team), the palliative unit at Hatanpää (ward and consultation team), the palliative unit in Valkeakoski (ward), the palliative unit in Sastamala (ward), the Pirkanmaa home hospital service, and as an affiliated unit, the Pirkanmaa Care Home.

Patients are admitted to the Palliative Care Center based on a referral or consultation from their attending physician. Palliative care is organized according to the needs of the patient and family, in line with the recommendations of the Ministry of Social Affairs and Health. Special level palliative care is provided in the units of the Palliative Care Center, while basic level palliative care (for example, Tays inpatient wards, local hospitals, and assisted living facilities) is supported through consultations and the services of the home hospital unit.

At the palliative care unit of Tays Central Hospital, a palliative outpatient clinic operates on weekdays. Patients are referred to the clinic by their attending physician. The clinic is staffed by two physicians and two nurses. The unit's tasks include, among other things, symptom‑relieving medical treatments and procedures, psychosocial support, and the creation of care plans.

Palliative outpatient clinic services are also provided in the palliative care units of Sastamala and Valkeakoski. In addition, Tays Central Hospital has a pediatric palliative care unit (outpatient clinic and consultation team).

The palliative care unit of the central hospital includes a psychosocial support unit that provides psychosocial counseling. Collaboration is carried out with Pirkanmaa hoitokoti and general hospital psychiatry. Depending on the situation, the services of psychotherapists from Pirkanmaa hoitokoti can be used for patients receiving palliative care and for their families.

The home hospital of the Pirkanmaa wellbeing services county covers almost the entire region. With the support of the home hospital, palliative care and end‑of‑life care can be provided at home. The home hospital also supports end‑of‑life care in assisted‑living facilities. Admission to the home hospital requires a referral from a physician, and the care provided is hospital‑level treatment at home. Long‑term care and support are provided by home care services. For palliative home hospital patients, the option to transfer to an inpatient ward (a so‑called support ward) is also arranged when needed.

The Tampere (Hatanpää) home hospital unit is open around the clock, while the other units operate for most of the day. In the wellbeing services county of Pirkanmaa, end‑of‑life care is provided in the wards of the palliative care center and the regional hospitals. For patients receiving palliative care at home, the possibility to transfer to a support ward is arranged if the situation requires it. The ward placement is organized with consideration for the needs of the patient and their family, as well as the patient’s place of residence. For patients living in assisted‑living facilities, end‑of‑life care is primarily arranged within their own housing unit, with support from the home hospital if needed.

The wards of the palliative care center are located in Hatanpää (Tampere), Sastamala, and Valkeakoski. Pirkanmaan hoitokoti (Tampere) serves as an affiliated unit of the center. Patients and families requiring demanding palliative care and end‑of‑life care are primarily admitted to the hospice and the center’s wards, but other patients may also be admitted when capacity allows. A significant amount of palliative care is provided on the wards of all local hospitals throughout Pirkanmaa. The palliative care center provides consultation support to all inpatient wards.

The palliative consultation teams of Tays Central Hospital and Tays Hatanpää support the palliative care and end‑of‑life care of patients treated on inpatient wards and in outpatient clinics. Each consultation team consists of a physician and a nurse specialized in palliative care. The consultation team of Tays Central Hospital provides telephone‑based palliative care consultations throughout the Pirkanmaa region, as well as in complex cases across the entire Inner Finland cooperation area.
