About My Path and digital pathways

The Health Village’s My Path service channel includes digital care pathways for health care and digital service pathways for social welfare, remote appointments and self-care programs open to all. It also offers the possibility to get help and support outside of traditional practices.

Health Village My Path is a digital service channel for health care and social welfare. My Path enables units to provide care and services to their patients via digital care pathways®. My Path is a secure online service that uses strong authentication. You can log in to My Path using your banking codes, mobile certificate or electronic certificate card.

NB: The Health Village’s My Path is one of the three main products of the HealthVillage.fi service package. The other products are the Health Village hubs and the HealthVillagePRO service. My Path has digital care pathways and self-care programs. You need a referral to access the digital care pathways, but the self-care programs are available to anyone registered for My Path without a referral.

Digital care pathways that complement traditional health care allow patients to

  • fill in treatment-related questionnaires

  • get information, peer support and treatment advice in the form of text, images, videos and audio files

  • do exercises and tasks related to treatment

  • send follow-up information on symptoms and health measurements

  • communicate with the professionals treating them via messages or remote consultations.

Digital service pathways complementing traditional social services allow clients to

  • get information on social services

  • do exercises

  • fill in surveys

  • send messages 24/7

  • communicate with a social worker at an agreed time

  • have a remote meeting with a social worker

  • read the frequently asked questions.

The digital care pathway allows a professional to manage patients or clients through a patient list, send them messages and create calendar events.

Digital care pathways in health care and digital service pathways in social services either complement or replace traditional consultations and transactions. The content of the digital pathways published in the My Path service has been produced by health care and social welfare professionals. My Path is a medical device.

When using the My Path, it is important to note that

  • The service providers are under no obligation to treat individual patients according to the contents or practices outlined on the website. Additionally, the service providers assume no responsibility for any subjective interpretations or applications of the service’s contents or instructions.

  • The duty of care always remains with the entity responsible for the person’s care.

  • The providers of the Health Village service cannot guarantee that the service works always without interruption or error. The service is subject to various periodic maintenance and updates, during which the service may be temporarily unavailable.

The content produced and reviewed by Finnish health care and social welfare providers is high-quality and safe health information.

Ownership, copyright and other related intellectual property rights to the content of the digital care pathways belong to the provider of the digital care pathway content: the HUS Group, the wellbeing services counties or third parties.

All rights to the website and its content are reserved. The publication, reproduction, transfer or modification of the content or layout of the website without the permission of the rights owner is prohibited.

The service provider aims to keep the information in the digital pathways and self-care programmes accurate and up-to-date and to ensure the technical functionality of the site. However, the accuracy, integrity and timeliness of the information contained in the service cannot be guaranteed in all situations.

Updates made by the service provider to the site’s layout and content, for example, for the duration of an update, may temporarily affect the availability of the site.

The service provider may change and update the descriptions, content or layout of the website without prior notice or without specific reason.

The service provider informs users of any disruptions to the service by means of an information banner at the top of the page or a separate downtime page.

The service provider may make changes to the website if necessary or restrict access to the website for compelling reasons. The service provider does not guarantee that the services can be used without interruption and is not liable for any damage caused by the use of the service.

Digital pathways and self-care programs contain links to websites operated by third parties. The service provider is not responsible for the content and accessibility of third-party sites.

Using the website

My Path works on the most commonly used terminals and browsers. Using My Path requires strong authentication. My Path may contain links to third-party services that may require you to log in to use them.
